You have to move your middle and your ring [Ab] finger [E] like this.
[G] So you notice my middle finger is moving from the D [Gb] note [G] to the G.

And you probably know an E [Gb] minor [Em] chord.
Now play that with your index and your middle finger on the B and the E note there.
Okay, now play the [Gm] 7th, which is a D, with your ring finger.

And you [G] notice that G note there, I'm just doubling up.

It's an A dominant 7th with a suspended [D] 4th.
[G] You probably know this chord here, A7.

Okay, now bring that C sharp up to a D.
